5 Headed Shark Attack is a Syfy original film produced by The Asylum, continuing the “multi-headed shark” franchise that began with 2 Headed Shark Attack and 3 Headed Shark Attack. This installment raises the stakes by introducing a monstrous five-headed shark, terrorizing the waters around Puerto Rico.

The story begins with a group of photographers and models working on a remote island shoot. Their session is abruptly interrupted when a bizarre four-headed shark attacks their boat, leaving chaos and bloodshed in its wake. Soon, the creature mutates into an even more terrifying form: a five-headed shark, with its multiple jaws snapping from all angles, resembling a deadly, living starfish.
Marine biologist Dr. Angie Yost is brought in by Thaddeus Marshall, the wealthy owner of the Puerto Rico Aquarium, to investigate the mysterious deaths and restore public confidence. Reluctantly joined by her interns and a rugged shark hunter named Red, Angie discovers that the threat is far greater than she imagined. The shark not only devastates tourist boats but also attacks military and police vessels, leaving the island in a state of panic.
As the survivors attempt to devise a plan, they use recordings of dolphin sounds to lure the predator into a trap. Tensions rise, alliances crumble, and sacrifices become inevitable. In the climactic sequence, one character gives their life to ensure the detonation of explosives that finally destroy the beast.
Despite its outlandish premise, the film embraces the over-the-top B-movie formula: exaggerated kills, absurd monster design, and tongue-in-cheek dialogue. While critically panned for poor acting and effects, 5 Headed Shark Attack delivers exactly what its target audience expects—outrageous shark chaos and campy entertainment.
